This form is used to collect company and contact information for Gateway, University Square North, University Square South and West Lane Avenue businesses that have a monitored third-party burglary or intrusion alarm system. This form does not apply to fire alarm systems.

Before submitting this form, please verify with your alarm monitoring company that The Ohio State University Police Department (OSUPD) is listed as the primary law enforcement agency for alarm response. Alarm monitoring companies should notify OSUPD at 614‑292‑2121 and select option 1.
